Why wrong AI answers happen (and why “tell the AI” usually fails)?
Most brand teams treat wrong AI answers like a customer support ticket: “We found an error. Please correct it.” But the system isn’t a database. It’s a probability engine.
OpenAI describes hallucinations as cases where a model confidently generates an answer that isn’t true, and explains that common training and evaluation procedures can reward guessing over acknowledging uncertainty. That’s why the same incorrect claim can keep resurfacing until the underlying evidence landscape changes.
So the fix is usually not arguing with the model. The fix is changing the sources the model (and the broader web ecosystem) can verify.
The workflow at a glance
- Capture the output (prompt, answer, date, screenshots, citations).
- Classify the error type (identity, drift, inference, relationship).
- Trace the likely upstream sources.
- Fix your canonical facts first (facts sheet + website + schema).
- Fix high-impact third-party contradictions.
- Strengthen identity anchors (sameAs, IDs).
- Monitor and re-test on a cadence.

Step 1: Capture the bad output like evidence
If you don’t capture it, you can’t measure it. Create a shared log (spreadsheet works) and record every incident. (you can also check with a tool like LLMtel.com)

Step 2: Classify the error (so you apply the right fix)
A) Identity error (entity merge)
AI mixes you with another company/person/product. This is a disambiguation problem.
B) Fact drift (stale info)
Old address, old CEO, old product line. This is usually an outdated source ranking well or a stale profile you forgot to claim.
C) Inference/hallucination
The system guesses a number, invents a relationship, or fills a gap with plausible-sounding nonsense.
D) Relationship confusion
Parent/subsidiary/partner relationships are misrepresented.
Step 3: Trace upstream sources (the reality check)
Start with the places that most commonly seed confusion:
- Your own site: About page, footer, Contact page, press releases, leadership bios
- Your structured data: Organization markup and any other entity markup
- High-authority profiles: LinkedIn, major directories, app marketplaces, review sites
- Knowledge bases: Wikidata/Wikipedia (if present)
- Earned media: articles that repeat outdated facts
If you find two different “truths,” the system will too. Fix contradictions before you chase the AI output.
Step 4: Fix your canonical facts first (owned truth)
- Update your Brand Factbook (this is your governance layer).
- Update your canonical About and Press/Media Kit pages to match.
- Update Organization structured data to match the visible content and approved facts.
- Avoid marking up facts you can’t maintain (accuracy matters).
Google’s structured data guidelines emphasize that structured data should not be misleading, should represent the main content of the page, and should be correct so treat your canonical pages and markup as the foundation.
Step 5: Fix high-impact third-party contradictions
This is the unglamorous part that produces disproportionate results. Prioritize the sources that prospects and journalists actually use.
Priority order (typical B2B)
- LinkedIn company page
- Top 3 industry directories (your buyers’ reality, not your vanity list)
- Google Business Profile (if relevant)
- Crunchbase/CB Insights style databases (if they influence your market)
- Review platforms (G2, Capterra, etc., depending on category)
For each profile: claim it, standardize the name, update the description to match your one-liner, and correct factual fields.
Step 6: Strengthen identity anchors with sameAs and entity IDs
Schema.org defines sameAs as a URL that unambiguously indicates identity. Google documents Organization structured data and supports sameAs as a way to connect your organization to other authoritative pages about it. Use it to help machines disambiguate you… carefully.
sameAs guardrails
- Include only official profiles and legitimate knowledge base entries.
- Do not include “mention pages.”
- Fix inaccuracies on external pages before linking to them.
- Keep the list short and high-confidence.
Step 7: Retest and monitor (turn this into a system)
Entity corrections often take time to propagate because crawlers, indexes, and models update on different schedules. Don’t guess, measure.
Monitoring cadence
- Monthly: run 10–20 standardized prompts and log results.
- Quarterly: audit top third-party listings for drift.
- Event-driven: re-test immediately after leadership changes, rebrands, acquisitions, or HQ changes.

PR/Comms playbook: severity levels
Not every wrong answer needs escalation. Use impact-based triage.

Optional but powerful: publish a public Brand Facts / Corrections page
For high-risk brands (regulated industries, public scrutiny, frequent confusion), publish a page that states your canonical facts plainly: Your founding year, HQ, leadership, category, and official profiles. Keep it aligned with your schema markup.
Common pitfalls
- Changing messaging in every channel (creates new variants for machines to learn).
- Fixing the AI output before fixing upstream sources (whack-a-mole).
- Linking to inaccurate profiles in sameAs (amplifies the wrong data).
- Treating Wikipedia like a profile you can edit freely (COI risk).
One-page checklist (print this)
- Log the error with prompt, answer, date, citations
- Classify: identity / drift / inference / relationship
- Check canonical pages: About, Contact, Press kit
- Check structured data: Organization markup present, valid, aligned with page content
- Check top third-party profiles: LinkedIn + key directories
- Fix contradictions (source-level), then re-test
- Add/adjust sameAs only after external anchors are accurate
- Monitor monthly, audit quarterly
